CCW Permit in Berkeley, CA?

How do I obtain a CCW Permit in the City of Berkeley, CA?
I would like to carry a Kimber CDP II, 45ACP, during warm weather and a Ruger Alaskan, 45LC, during colder weather.

When you conceal carry, do you also carry a backup sidearm? (Not including a knife).
