Passport required for CCW in KY? I was reading over the form to apply for a CCW permit and it asks for a Passport number on question 8. Is this required to obtain a CCW in KY? It doesn't say all fields required or skip if you don't have on it so i am at a loss and can't find the answer anywhere

"If your answer to Question 2 was yes, you do not need to respond to Questions 3-15 below"
RECAP:
If you are a U.S. citizen you DO NOT NEED TO RESPOND to questions 3-15. Only NON CITIZENS need to supply a passport number.
If you are a U.S. citizen, answer question #1, answer question #2, have it notarized and send it in.

Wow i read that thing over 3 times how the F did i miss that. Thanks for that, i can't believe my oversight.
It is good to know its a quick process though.
I don't remember ever having to even fill out that form when I applied for my ccdw. After you take your ccdw class and get your certificate you just need to go to your local sheriffs office and fill out the paperwork. Your ccdw insructor should supply you with what info and documents you need. The lady at my sheriffs office filled everything out for me and all i did was sign my name. Good luck. Btw don't get discouraged waiting on your permit.....couple of guys that went to the same class as i and sent in there applications the same day got their permits 2 weeks before i got mine. No known reason just the way it goes. Good luck
I guess they sent in your affidavit for you, but its required by state law. They must have been a notary as well. Check the form i uploaded in the first post, is it the same one you just filled out name and signed?Originally Posted by d-buck
