I wear suspenders and a gun belt and carry OWB. I also went from carrying a Colt Combat Commander to carrying a Colt New Agent.

I've got crushed discs, scoliosis, degenerative spinal disc, arthritis, and more I don't want to think about up and down my back. I OWB a Beretta 92FS in a Blackhawk Serpa and an XD(m) Compact 3.8 in a Galco Fletch. A really good gun belt makes all the difference for me for OWB. 50% of the time I carry each in a Galco Miami Classic shoulder rig, properly balanced of course. I decide what to carry and with what based on a lot of things but pain is a factor. If my lower back hurts worse, I use the Classics. If upper back is the bigger problem that day, it's OWB using a Blackhawk Instructor's gun belt.

I'm still a young man so no arthritis or other situations typically age related, but I have my fair share of back pain. I came into the world with 6 lumbar vertebra rather than the standard issue of 5. This tends to put a great deal of undue stress on my lower back. My pain level constantly varies from non existent to moderate ache and eventually unable to move at all without extreme discomfort. I carry IWB 90% of the time with either a galco summer comfort or crossbreed supertuck. When I'm having back pain I will normally switch to OWB blackhawk or just opt to not carry at all. When my pain is at its worst I can't get out of bed anyway.
I'm not sure this helps at all, but I thought I'd weigh-in. I hope you find a method of carry that works well for you, but nothing wrong with changing it up when necessary. Whatever you decide be sure to practice all of them thoroughly.

pheasantd24, I have major back and neck issues, 2 plates/fusion C-5/6/7 and I have had lamicectomy's at L-3/4/5 with another surgery pending for a bone spur at L-5. As amazing as it sounds I have had no issues with carrying IWB, G23 in a mitch rosen ARG. This is a comfortable rig, I think the key is the double thick belt belt I wear, as it is very rigid and supports the weight well. Also the ARG is not a bulky holster due to where the belt loop is. I hope you find a rig that is comfortable for you. The Crossbreed is very popular, I have heard nothing bad about them.

wizard Thank you very much. I should receive my Crossbreed IWB soon. I cant wait to try it. Wizard I hope all is well with your back issue I know how you feel. It is hard that with a back problem people see us as not disable due to the plain fact they can not see our injury until we take off our shirts. Which I DO NOT DO. Take Care And wish you well
