I have a herniated L5/S1 disk and carry both depending on my clothing on a given day.

I have lumbar spinal stenosis and find OWB much more comfortable than IWB with my Springfield Pro.

I have had a severe muscular tear recently and carry OWB at the 4-5 o'clock position. Cross draw is pretty comfortable too.

I have hardware at L3-4 to L5-S1, screws, spacers, cables, I find it impossible to carry in the back area at all. My prefered carry is right above the front pocket on my jeans RH (strong side). Doesn't pull my pants down either and my belt doesn't have to (can't) be real tight. I am relatively thin so If I carry at 3 o'clock it seems very noticable with a tucked shirt, carrying slightly foreward makes a big difference. I don't have any issues trying to draw while sitting either, or driving for that matter. I usually carry IWB
G36, KHOLSTER. Have also carried S&W 1911 ES in a supertuck same spot good results.

Maybe try suspenders? Anyone know if they help distribute the weight?
God gets blamed for a lot of things now days, they ask “why would He let this happen?, Why doesn’t He stop it? I really don’t know, but I wonder if He wants to ask us the same questions. All that is necessary for evil to triumph is for good men to do nothing

Wow. Lots of screwed up people here, wish I was a chiropractor, could make a couple extra bux. :-) I guess it would be easy to say no one carries in the s.o.b. then.

I set my CB up with a forward cant and keep it at the 3 o clock position. It is very comfortable this way. Hope that helps.

Oh, and 1 size bigger pants than I normally use.

Back problems haven't gotten to the point of surgery, yet. Neck is a whole different story. Shots to the lumbar area have done the job of controlling the pains so far. That and a 20# weight restriction. I carry a 1911 ( Taurus PT 1911 SS or Sig C3, both in .45 ) IWB at about 0200. I use an adjustable web belt ala US Coast Guard.
