    They did say it was not another officer's fault.
    Rule #2 - Never point the muzzle in any direction that should a discharge occur a human injury would result.

    One point we can drive home with this is it was an "Instructor Course" so they were experienced shooters. We do not know the details but as SGB pointed out Rule number 2 may have been broken.
