Just about any site online where we register to join they send us a confirmation email, to which we then respond. It takes just a few minutes. I see little difference between doing that and joining a group here in order to post in that group. It's a non-issue for me.

I would think a better poll would be the 3 choices above: Okay/Not okay/Don't care and make it so a viewer could not read the thread without clicking one of the buttons. I don't know if it is possible to do that, would be interesting to see how many threads got viewed!

I do believe that the "Don't care" buttons would be clicked a large percentage of the time.

The groups is a good idea and a little check-out, (i.e. vetting by Canis Lupus kind of thing) is perfectly fine. Why not keep group-specific groups specific as there are plenny o' other places for others.
So that's a hand up in the air in this corner for groups w/memberships.
